Parent Consent and Liability Waiver
IMPORTANT NOTICE FOR THE LEGAL GUARDIAN OF A MINOR
PLEASE REVIEW THIS DOCUMENT THOROUGHLY. BY SIGNING, YOU CONSENT TO YOUR CHILD’S PARTICIPATION IN ACTIVITIES THAT POSSESS INHERENT HAZARDS. YOU ACKNOWLEDGE THAT DESPITE THE DILIGENT CARE PROVIDED BY CROOKED PALM COLLECTIVE LLC, THERE REMAINS A RISK OF SEVERE HARM OR FATALITY DUE TO UNAVOIDABLE DANGERS NATIVE TO THE WILDERNESS ENVIRONMENT. YOU ARE ELECTING TO FORFEIT THE LEGAL RIGHTS OF YOURSELF AND YOUR CHILD TO SEEK FINANCIAL RECOVERY FROM CROOKED PALM COLLECTIVE LLC FOR ANY BODILY INJURY, LOSS OF LIFE, OR DAMAGE TO PERSONAL ITEMS ARISING FROM THESE NATURAL RISKS. WHILE YOU RETAIN THE OPTION TO DECLINE TO SIGN THIS WAIVER, CROOKED PALM COLLECTIVE LLC RESERVES THE CORRESPONDING RIGHT TO WITHHOLD YOUR CHILD’S ELIGIBILITY TO JOIN THE PROGRAM WITHOUT A DULY EXECUTED CONSENT FORM. PLEASE ENSURE YOU FULLY COMPREHEND THESE TERMS BEFORE PROCEEDING FORWARD.
1. Participation and Activities
I give permission for my child to participate in supervised Crooked Palm Outdoor Academy classes, activities, special events, and field trips conducted at Crooked Palm or another designated location.
Activities may include outdoor play, running, climbing, movement activities, nature exploration, gardening, arts and crafts, woodworking, supervised use of age-appropriate hand or power tools, shelter building, fishing, kayaking, boating, swimming, water activities, outdoor cooking, fire-safety instruction, animal encounters, farm activities, archery, and visits to parks, nature centers, museums, farms, springs, zoos, or other educational locations.
Information about each field trip, including its location, schedule, transportation arrangements, and any special requirements, will be provided to parents in advance.
2. Acknowledgment of Risk
I understand that outdoor, hands-on, and field-trip activities involve inherent risks that cannot be completely avoided.
These risks may include falls, cuts, splinters, burns, tool-related injuries, sprains, broken bones, heat illness, dehydration, severe weather, insect or tick bites, stings, allergic reactions, irritating or poisonous plants, animal encounters, uneven or slippery terrain, water hazards, drowning, transportation-related risks, and the actions of other participants or members of the public.
I understand that conditions and risks may vary depending on the activity and location. Crooked Palm will use reasonable safety precautions, supervision, protective equipment, and age-appropriate instruction.
I agree that my child must follow all safety rules and instructor directions. I understand that Crooked Palm may modify or end my child’s participation in an activity if staff believe it is necessary for safety.
3. Assumption of Risk and Release
I voluntarily allow my child to participate and accept the inherent risks associated with Crooked Palm classes, activities, special events, and field trips.
To the fullest extent permitted by Florida law, I release Crooked Palm Outdoor Academy, the property owner, and their respective owners, employees, instructors, volunteers, contractors, and agents from claims for personal injury, death, or property damage resulting from the inherent risks of the activities.
This waiver does not release claims that cannot legally be waived under Florida law.
4. Emergency Medical Permission
If I cannot be reached during an emergency, I authorize Crooked Palm staff to provide basic first aid, contact emergency services, and obtain reasonably necessary emergency medical treatment for my child.
I authorize emergency medical transportation when reasonably necessary. I understand that I am responsible for medical expenses incurred on my child’s behalf.
